29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Job Site:  On or about 9/21/2012, employees were performing roofing work on a ten (10) pitch roof and were not provided with fall protection.    b) Job Site:  On or about 9/21/2012, employees were performing roofing work on an eight (8) pitch roof and were not provided with fall protection.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or when such an extension is not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided:    a)  Jobsite:  On or about, 9/21/2012, the two portable extension ladders were not extended 3 feet above the landing surface of the sloped roof and were not provided with a rigid support  and grabrail at the top when these ladders were being used by the employees performing roofing work.
