29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  Coffee Roasting: The lockout/tagout program did not include machine specific procedures for all equipment including but not limited to the rotary airlock feeder when there is a coffee chaf back up and the seamer when there is a cover jam.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:  a) Coffee Roasting: The rotating part on the rotary airlock feeder was not adequately guarded.   On 11-5-12  an employee suffered a finger amputation after reaching into the unguarded rotating airlock lock feeder.    b) Canning: Rotaing shafts on the seamer machine were not guarded.
