29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:        JOB SITE: 2006-2012 Running Ridge Court, Maryland Heights, MO 63043, at this work site, workers were observed top of the apartment complex, located on the north end of the building, approximately 18 feet 3 inches eave to ground level, actively install new roof shingles, operating pneumatic nail guns without any form of eye or face protection.  The workers were exposed to a struck-by hazard.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:      JOB SITE: 2006-2012 Running Ridge Court, Maryland Heights, MO 63043, at this work site, located on the north end of the building, a worker was observed climbing an aluminum extension ladder, while carrying a large box of material and did not have least one hand when progressing up the ladder.  The worker was exposed to a fall hazard.
