29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    5640 Read Blvd, New Orleans, LA; The employer does not instruct each employee in the recognition and avoidance of unsafe conditions. On or about December 19, 2012, an employee was demolishing a six (6) story buliding using an American Crane, Model number 999, s/n GS9928. The employee was not trained to recognize the hazard of being struck by falling debris when working too close to the structure and keeping the cab windows open.

29 CFR 1926.1501(a)(12): 29 CFR 1926.1501(a)(12) all windows in cabs were not of safety glass, or equivalent, that introduce no visible distortion will interfere with the safe operation of the machine.     5640 Read Blvd., New Orleans, LA.; On or about December 19, 2012 the employer allowed employee(s) to use an American crane, Model number 999, s/n GS9928, with a cracked window on the cab door. The window obstructed the view of the employee.
