29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    (a) At the Garden Avenue work site employees worked at a height of 25 feet above ground level on a sloped residential roof without benefit of fall protection.  Employees accessed the roof at a level 17 feet above ground from an extension ladder that had not been erected properly extending at least 3 feet above the roof landing.  Other hazards included lack of eye protection, lack of head protection, and protruding nails .  Regular inspections by a competent person would have identified these hazards and corrected these hazards.    Abatement verification is required for this violation.
