Throughout facility - On or about December 27, 2012, the unused portion of the blade of the vertical DoAll bandsaw, model # ML, serial # 4610333, were not enclosed or guarded, thereby exposing employees to the hazards associated with reciprocating blades.      f. Throughout facility - On or about December 27, 2012, the unused portion of the blade of the horizontal Wells bandsaw, model #58B serial #9157, were not enclosed or guarded, thereby exposing employees to the hazards associated with reciprocating blades.

29 CFR 1910.303(c)(3)(i): Conductors shall be spliced or joined with splicing devices identified for the use or by brazing, welding, or soldering with a fusible metal or alloy. Soldered splices shall first be spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.    Maintenance Department - On or about December 27, 2012, the employer failed to insulate conductors with an insulating device identified for that purpose on the Miller Welder model number CP-300, serial number JG060089, thereby exposing employees to electrical hazards.
