29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a/  At the job site, one employee was working from a tubular weld standard frame scaffold that was two tiers high.  The working platform consisted of two 2 X 12 planks.  This scaffold was capable of supporting five such planks.

29 CFR 1926.451(b)(5)(i):     The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilivered end:    a/  At the job site, one employee was working from a tubular weld standard frame scaffold that was two tiers high.  Both ends of the two 2 X 12 planks consisting of the working platform extended over the horizontal suuports by approximately 29 inches.
