In the south side of the building, employees were using an extension cord with a missing ground prong, thereby being exposed to a contact hazard. b. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the south side of the building, employees were using an extension cord with the plugs strain relief pulled away from the cord exposing interior conductor insulation, thereby being exposed to a contact hazard. b.
