29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    a) On or about November 7, 2012 and times prior, fall protection for plastic rail cars; The employer failed to ensure employees climbing onto grey rail cars to check seals were provided fall protection since these rail cars do not have guardrails installed on top around hatches exposing them to fall hazards of approximately 12 feet.
