29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) Hwy 31 in Bay Minette, AL:  On or about January 7, 2013 and prior to, the employer exposed employee(s) to a 15 foot fall hazard while performing electrical light fixture replacement from an aerial lift.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    a) Hwy 31 in Bay Minette, AL:  On or about January 7, 2013 and prior to, the employer exposed employee(s) to a 15 foor fall hazard in that scaffold fall safety training was not provided to employees while performing electrical light fixture replacement from an aerial lift.
