29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). Jobsite: A Werner aluminum extension ladder utilized to in a stairwell which was 41 inches wide was extended to 15 feet 7 inches not allowing for the four to one distance ratio.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Jobsite: Employees using extension ladders were not given training in all OSHA requirements for use of ladders and procedures to be followed to minimize hazards.
