North West Corner of the Liquid Filling Department/around column # L18 - On or about January 4, 2013, an amployee was required to operate a chemical process at 7.9 feet (95") from the ground by utilizing portable metal ladders that were not in good usable conditions. The conditions of the portable metal ladders were as follows;    a. The steel (roll-safe-lock) ladder with 10 steps showed metal damaged by having a twisted and turned piece at its base and a twisted section on one step. Exposing employees to a fall hazard.    b. The tilt and roll ladder with 3 steps was missing its rubber pads and showed signs of instability. Exposing employees to a fall hazard.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998].    a. Liquid Filling Department/South West Corner/Column # L18 - On or about January 4, 2013, an employee was splashed with a liquid corrosive mixture (such as sodium hydroxide and 2 - butoxyethanol) while transferring the liquid contents from one container to another container. The employer did not provide employees with suitable facilities for quick drenching of the eyes and body within the work area for immediate emergency use. Employees were thereby exposed to the hazards of chemical burns.      THE EMPLOYER, J. F. DALEY INTERNATIONAL dba DALEY INTERNATIONAL, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.151(c) WHICH WAS CONTAINED IN OSHA INSPECTION # 312599152, CITATION NUMBER (2), ITEM (2), AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 10, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 4100 W. 76TH ST. CHICAGO, IL 60652.

Employees were thereby exposed to the hazards associated with chemical burns.
