29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Located at 2 color press employees were not protected from crushing hazards where the loading elevator terminated at floor level. Guarding was not provided to prevent employees from entering there body or limbs into the space below the elevator when elevated while the elevator was operational. (b) Located at the die cutter model CO1PM03 employees were exposed to points of operation at the top, sides, and front where could easily place there hands. Abatement certification and abatement document is required for this violation. The documentation should include written verification of abatement, applicable measurement or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
