29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Kecy Corporation, Wauseon, Ohio: On or about November 8, 2012, the employer did not ensure that employees were protected from hazards associated with the Colchester Lathe Co. Ltd. manufactured 15 lathe. The lathe was not adequately guarded to protect employees from flying chips and rotating parts of the chuck.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Kecy Corporation, Wauseon, Ohio: On or about January 7, 2013, the employer did not ensure that the left side of the Black and Decker 8 inch bench grinder was equipped with a work rest that was adjusted to 1/8th inch away from the abrasive wheel. Employees that operated the grinder were exposed to injuries from potential jammed work and/or breakage of the wheel.

The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operator's knowledge. a) Kecy Corporation, Wauseon, Ohio: On or about January 8, 2013, the employer did not ensure that the operator(s) of the Minster 200 tonnage mechanical power press #263 was instructed in the safe operation of the press in that the key selector for the press run mode was not supervised. The key, used to change press modes, was kept in the selector switch and not supervised to assure that the press mode was not changed without the operator's knowledge.
