29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized: a) Located along the exterior area of the structure an employee was observed within the basket of a vehicle mounted aerial lift, not otherwise personally electrically isolated performing 110v lighting system fixture removal, conductor rewire and fixture replacement associated operations on which the interior structure's deenergized panelized breaker was not affixed with a tagout tag and/or lockout tagout device to minimize reenergization of the electrical service.

The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated a) Located along the front exterior area of the structure there were two employees oberved performing electrical lighting fixture replacement operations from within the basket of a vehical mounted extensible and articulating boom aerial lift without a harness / safety belt on or lanyard attached to the boom or basket and were exposed to fall potentials in excess of 20'.
