29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas. On or about January 8, 2013, inside the building, a pile of debris had accumulated inside the work area exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about January 8, 2013, on the north side, employee were exposed to fall hazards of approximately 19 feet while engaged in framing work on a residential construction site.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided. On or about January 8, 2013, on the north side, the employer did not provide an extension ladder where the employees could access and egress the upper landing exposing the employees to fall hazard.
