29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about December 20, 2012, Kampmann Roofing, as a controlling employer, did not ensure that Alvaro Rosales (subcontractor) protected his employees from head injuries while they worked without head protection in areas where shingles and other roofing waste were being thrown from upper levels.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about December 20, 2012, Kampmann Roofing, as a controlling employer, did not ensure that Alvaro Rosales (subcontractor) protected his employees from fall hazards by providing a ladder or other adequate means of access between roof levels. To reach the second level of a roof which was approximately 5 feet above the first level, employees were observed grabbing the rake edge and pulling themselves up and over.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about December 20, 2012, Kampmann Roofing, as a controlling employer, did not ensure that Alvaro Rosales (subcontractor) protected his employees from falls greater than 6 feet while performing roofing work without adequate fall protection measures in place. Kampmann Inc., dba Kampmann Roofing was previously cited for a violation of Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection 313714842, Citation 1, Item 1, issued November 3, 2009, with respect to a workplace located at 440 S. Cody Ct., Lakewood, CO 80226. This citation became final on January 25, 2010.
