29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)    On or about January 9, 2013, employees were exposed to chemical hazards when the employer did not provide employees information on the location of the program and MSDS or employee training.  Employees were exposed to chemicals such as, but not limited to, dust generated when mixing and using mortar containing Portland cement.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about January 9, 2013, employees were exposed to struck-by hazards when the employees did not use safety glasses while installing rock on the front of the house.    The employees were breaking rocks up with pointed tip rock hammers in order to fit the rocks into the facade.

On or about January 9, 2013, employees were exposed to fall hazards of approximately 4.4-6.5 feet when there was no ladder to access the scaffold located in front of the front porch.
