29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a) 600 S. Pine St., Deridder, LA; the employer failed to ensure each level of a scaffold was fully planked. The first level of the scaffold had gaps between the planks up to 1 1/2 feet wide. The second level of the scaffold was missing two planks on part of the level. These conditions exposed employees to a fall hazard of up to 13 feet.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold shall be protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a) 600 S. Pine St., Deridder, LA; the employer failed to ensure that a scaffold employees were working from was equipped with a complete guardrail system. The top level of the scaffold was missing the top and intermediate rail. The scaffold was 13 1/2 feet high. Employees were exposed to a fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) 600 S. Pine St., Deridder, LA; the employer failed to ensure a scaffold used for overhand bricklaying was equipped with a ladder for access and egress. The work platform was approximately 13 feet above the ground.

Employees working underneath were exposed to a struck-by hazard.
