29 CFR 1910.217(c)(1)(i):     The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    (a)  Employee(s) operating a Federal 100 ton mechanical punch press in the Tie Press department were exposed to ampuation hazards due to a damaged guard allowing access to within 10 inches of the point of operation.  The damaged guard had a 6-1/2 inch by 7 inch opening that allowed access to the point of operation.     (b)  Employee(s) operating Iron worker #1 in the Fabrication Department were exposed to amputation hazards due to no point of operation guarding being provided for employee(s) using their hands to place materials at the point of operation.    (c)  Employee(s) operating Iron worker #2 in the Fabrication Department were exposed to amputation hazards due to no point of operation guarding being provided for employee(s) using their hands to place materials at the point of operation.    (d)  Employee(s) operating the Side Rail Notch mechanical punch press in the Fabrication Department were exposed to amputation hazards due to no point of operating guarding for the right or left side of the machine where a notch was punched on the side of metal rails.
