29 CFR 1926.451(c)(2)(v):     The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    (a)   Front of Building - On or about January 8, 2013 an employee was exposed to an 8 to 9 foot fall hazard while elevated on a scaffold platform by a rough terrain forklif. The wood pallet was not secured to the forks.
