Dixie Hwy. Bowling Green, OH: The employer failed to ensure employees were protected from falls while performing roofing work on a steep sloped commercial type structure. Employees working in this condition were exposed to falls of approximately 10 ft. to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step. (a) John Smagola on or about January 10, 2013, at the address of 17801 N. Dixie Hwy. Bowling Green, OH: The employer failed to ensure the top of an 8 ft. self-supporting ladder was not used as a step. An employee was utilizing the top of the ladder as a working surface to reach the roof level during shingling work. The employee was exposed to fall hazards due to improper ladder use.
