29 CFR 1926.501(b)(13):  Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)Worksite, 2697 Covered Bridge Rd.,Merrick- employees were installing a roof with a pitch of approximately 8-12 and  working at a height of approximately 18 feet without fall protection; on or about 01/09/13.     Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a) Worksite, 2697 Covered Bridge Rd., Merrick- employee was exposed to electric shock while working within several inches of an unprotected and energized 220v powerline leading to home from street electric pole. on or about 01/09/13.      Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:    a) Worksite, 2697 Covered Bridge Rd., Merrick- employee was working on a 32' aluminum extension ladder less than 12 inches from an energized 220v powerline. The aluminum ladder did not have non conductive siderails; on or about 01/09/13.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
