29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    On or about 8/9/12    Location: 156 Central Avenue, Hartsdale, NY 10530    a) The female plug end of a yellow extension cord was damaged. The outer insulation was seperated from the plug exposing the insulated conducting wires..     b) The cord on the Hilti 1400-PE recipercating saw was damaged. The outer insulation was seperated from the plug exposing the insulated conducting wires..
