29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:       (a)  On or about 1/11/13, on the south end of the building, the second level of the scaffold platform was not fully planked, exposing employees to fall hazards of more than 10 feet to the concrete below.

29 CFR 1926.451(c)(2):  Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:      (a)  On or about 1/11/13, on the south end of the building, the scaffold legs did not have base plates, exposing employees working on the scaffold to falls of more than 10 feet to the concrete below.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    (a)  On or about 1/11/13, on the south end of the building, a ladder was not provided for employees to use to access the second level of the scaffold.  Employees were exposed to fall hazards of more than 10 feet to the concrete below.

29 CFR 1926.451(g)(1)(vii):  Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:        (a)  On or about 1/11/13, on the south end of the building, an employee was working near the end of the second level of the scaffold without being protected from falling to the concrete below by having a guardrail or other type of fall protection provided.  The employee was exposed to fall hazards of more than 10 feet to the concrete below.
