29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a) 183 Turnpike Rd Westborough, MA: On or about January 14, 2013, three employees were observed working on this steep roof more than 15 feet above the ground below without any fall protection as required by this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    (a) 183 Turnpike Rd Westborough, MA : On or about January 14, 2013, employees were using 2 seperate extension ladders to access the roof, neither of which extended any distance past the roof surface as required by this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
