29 CFR 1910.176(b): Storage of material created a hazard:    In the electrical / janitor room: The employer had employees stack up boxes of old Pamida plastic shopping bags on a constructed storage shelf. The boxes were not stacked neatly and were leaning, the shelf was bowed and appeared to be lower on the left (north) side.

29 CFR 1910.303(b)(7)(i): Unused opening in electrical cabinets were not effectively closed provide substantially equivalent protection to the equipment:    On multiple support columns in various departments in the store: Electrical outlets used by employees for purposes such as but not limited to vacuuming were missing knock outs exposing internal electrical conductors.

29 CFR 1910.305(g)(1)(iv):     Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1):    In the back office area of the store: The employer allowed the use of an extension cord as a substitue for permanent power. The cord was plugged in to an outlet in the office area and ran through the ceiling and over to the east storage room.
