Location: Press Area, 1100 Boston Avenue, Building #3, Bridgeport, CT 06610    The two hand controls on the #18 Stokes Pennwalt 20 ton partial revolution mechanical power press were not concurrent thereby requring concurrent pressure from both hands during the die closing portion of the stroke.

Location: Press Area, 1100 Boston Avenue, Building #3, Bridgeport, CT 06610    The 4 inch wide, 5 inch long opening within the point of operation barrier guard 9 inches from the point of operation on the Gasbarre Products 60 ton partial revolution mechanical power press did not conform to the O-10 table in that the maximum barrier guard opening allowed is 1 1/4 inches.
