29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 336 Northside Drive, NW, GA (Shop Area)- Authorized employees were exposed to unexpected energy start-up hazard. The employer did not develop and implement a lockout/tagout program, which consists of energy control procedures for each electrical hardwired machine, employee training, and periodic inspection. 29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use: a. 336 Northside Drive, NW, Gainesville, GA (Developer Room) - Employee was exposed to chemical hazard. An employee is required to handle and/or change out the corrosive containers containing sodium hydroxide that go into the developer machine for removing the coating off the plates. There was no suitable eyewash/shower station located in the immediate area for emergency use, on or about 01/15/13.

A written hazard communication program had not been developed to promote the safe handling and use of the hazardous materials, on or about 01/15/13.

Several chemical 1 liter bottles were not labeled as to their identification and content. The unmarked/unlabeled portable plastic bottles were filled with solution including but not limited to Velocity plate cleaner, Omega plate cleaner, water-based ink, Eezy-Klene, developer solution, Power Wash, Repro-Kleen and Magnitek-Ko-I-Wash, on or about 01/15/13.
