29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) On or about 01/17/13, in the establishment, employees were involved in servicing and maintaining equipment, such as, but not limited to: changing a gear box and repairing indexer/multiprinter machines, without using an energy control program, machine-specific lockout-tagout procedures, hardware, training or periodic inspections. While performing maintenance on indexer/multiprinter machine#1, employees only used a tag on the circuit breaker instead of applying a circuit breaker lockout panel device with a padlock.
