29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about January 17, 2013, employees performing framing activities working at upper levels were using lumber, nail guns, circular saws, hammers, and other hand tools and the employees working at lower levels were not wearing protective helmets to protect the employees from head injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about January 17, 2013, employees performing framing activities were not protected from falling by personal fall protection or any other protective systems.  Employees were exposed to a fall hazard of approximately 22 feet to the ground below.
