29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about January 17, 2013, at the above address site:  An employee was exposed to a fall hazard while performing electrical work ensuring ground connections were made while on a walking/working surface with unprotected edges approximately 11 ft. 2 inches above a lower level.

29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:      On or about January 17, 2013, at the above address site:  Employees gaining access to and from a walking working surface approximately 11 ft. 2 inches above a lower level using an aluminum extension ladder set up outside an active doorway with worker foot traffic did not have that ladder secured to prevent accidental displacement.

PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
