29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. On or about January 17, 2013, on the north side of the construction site, a pile of debris had accumulated near the ladder where employees climbed onto the roof, exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about September 6, 2012, employees working from a roof were not protected from fall hazards of more than 11-feet to the lower level.
