Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear: On or about 01/17/13, at 525 FD Roosevelt Avenue, Suite 1202, San Juan, PR, in the Investigator's Office #1, an electrical outlet box was not covered by a faceplate or any other means.
