An employee working as an abrasive blaster was exposed to a noise dose of 103.5%, or an equivalent dBA of 90.2 dBA, which was capable of causing permanent hearing loss. The sampling was performed for 297 minutes during one shift on January 31, 2013. The employer did not have a continuing, effective hearing conservation program.

On or about January 15, 2013, in the open painting bay, on the sidewall near the front of the bay, a wall mounted electrical receptacle box used to power a radio was within 20 feet of the spraying area, exposing the employee to fire hazards. b. On or about January 15, 2013, in the open painting bay, on the sidewall near the back of the bay, a wall mounted electrical receptacle box used to power a floor fan was within 20 feet of the spraying area, exposing the employee to fire hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED
