29 CFR 1910.134(c)(1):     A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) Roccos Collision Center: Employees (Welder and Painter) were provided with and were required to wear a 3M 07193 half-face tight-fitting elastomeric respirator while welding and painting vehicles. The employer did not develop or implement a written Respiratory Protection Program for those employees required to wear respiratory protection. On or about January 10, 2013.

29 CFR 1910.134(e)(1):     The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:      a) Roccos Collision Center: Employees (Welder and Painter) were provided with and were required to wear a 3M 07193 half-face tight-fitting elastomeric respirator while welding and painting vehicles. The employer did not ensure that employees were medically evaluated and able to wear the respirators without medical concerns. On or about January 10, 2013.
