29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) 1152 Harrisburg Pike, Carlisle PA 17013 - Employees were observed without fall protection while sheathing a roof and were exposed to a 30 foot fall hazard to the ground below, on or about January 18, 2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) 1152 Harrisburg Pike, Carlisle PA 17013 - Employees were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas, on or about January 18, 2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) 1152 Harrisburg Pike, Carlisle PA 17013, 2nd Floor - Guardrails used as fall protection at wall openings throughout the 2nd floor were observed without midrails, exposing employees to an approximate 10 foot fall hazard to the ground below, on or about January 18, 2013. b) 1152 Harrisburg Pike, Carlisle PA 17013, 3rd Floor - Guardrails used as fall protection at wall opening throughout the 3rd floor were observed without midrails, exposing employees to an approximate 20 foot fall hazard to the ground below, on or about January 18, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
