29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:     a) Jobsite, Montour Athletic Center, 223 Clever Road, Mc Kees Rocks, PA: On or before 01/22/13, two employees were inside the basket of a Terex TB 60 aerial lift elevated to a height of 31 feet from the ground level and were not wearing a personal safety harness and lanyard attached to the basket.      The T E Maugans Co.was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard 29 CFR 1926.453(b)(2)(v) which was contained in OSHA Inspection number 311474118, Citation number 01, Item number 01, issued on 02/26/2008, with respect to a workplace located at 1995 E. State St., Salem, OH.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Jobsite, Montour Athletic Center located at 223 Clever Road, Mc Kees Rocks, PA: On or before 01/22/13, two employees were working on the roof of the building installing soffit and metal panels and removing scrap material.  The employees were working within 3 feet of the edge of the roof and exposed to a 31 foot fall hazard.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a) Jobsite, Montour Athletic Center, 223 Clever Road, Mc Kees Rocks, PA: On or before 01/22/13, the employer did not have a written certification of fall protection training for the 2 employees working on the roof of the building installing soffit and metal panels, and removing scrap material. The employees were working within 3 feet of the edge of the roof and exposed to a 31 foot fall hazard.
