29 CFR 1910.147(c)(4)(ii):     Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    Shoal Creek Shavings, Cleveland, GA - Employee(s) performed maintenance and repair on equipment such as but not limited to the wood shaver and the cut off saw. The wood shaver had two energy sources, electrical and hydraulic. Specific lockout/tagout written procedures were not developed, on or about 1/24/13.

29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    Shoal Creek Shavings, Cleveland, GA - Employee(s) performed maintenance and repair on equipment such as but not limited to the wood shaver. When two employees were performing maintenance to the wood shaver only one lock was applied by one of the employees. Each one of the employees did not lockout/tagout the equipment, on or about 1/24/13.

29 CFR 1910.95(k)(1):     The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:    Shoal Creek Shavings, Cleveland, GA - An employee operating the two box wood shavers was exposed to noise at a TWA of 98 dBA, exceeding the action level of 85 dBA. The sample was taken over 173 minute period. Zero was assumed for the unsampled period of 307 minutes. Hearing tests had not been conducted, on or 1/24/13.    Shoal Creek Shavings was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.95(k)(1), which was contained in OSHA inspection number 447633, citation number 1, item number 1c and was affirmed as a final order on 10/16/12, with respect to a workplace located at 5457 Westmoreland Rd, Cleveland, GA.
