29 CFR 1910.178(l)(1)(ii):     The employer did not ensure that each operator had successfully completed the training required prior to permitting an employee to operate the powered industrial truck:  a) Lot 8 Olive Way, Milton, DE -  The employer did not ensure that the employee operating the Gradall Material Handler (Powered Industrial Truck), Model G6-42P, Serial # 0189175, was trained to operate the equipment. This exposed the employees to a struck-by hazard. This violation was observed on or about December 14, 2012.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Lot 8 Olive Way, Milton, DE  Employees were exposed to a fall hazard greater than 6 feet when they were installing roof trusses while standing on wall plates without the use of any PFAS. The employees were approximately 8 feet above the next level. This violation was observed on or about December 14, 2012.
