This violation was observed on or about January 22, 2013, in the machine shop area where employees were exposed to a struck-by hazard when operating a Mori Seiki CNC Lathe (Model ZL  250, Employer ID 1480-1) without the door interlock switch being active.    b. This violation was observed on or about January 22, 2013, in the machine shop area where employees were exposed to a struck-by hazard when operating a Mori Seiki CNC Lathe (Model ZL  203, Employer ID 1480- 2) without the door interlock switch being active.    c. This violation was observed on or about January 22, 2013, in the machine shop area where employees were exposed to a struck-by hazard when operating a Mori Seiki CNC Lathe (Model ZL  253, Employer ID 1480- 3) without the door interlock switch being active.    d. This violation was observed on or about January 22, 2013, in the machine shop area where employees were exposed to a struck-by hazard when operating a Mori Seiki CNC Lathe (Model ZL  253, Employer ID 1480- 4) without the door interlock switch being active.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that machines are guarded to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
