29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, at the front of the home being built, an excessive amount of debris such as but not limited to loose brick pieces, various size wooden boards and empty 5 gallon plastic containers were scattered on the ground. This condition exposed employees to the hazard of falls from the same elevation.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access: On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, on the supported scaffold located at the front of the home being built, employee dismounted from the 1st level platform and climbed down the end frames to lower ground level. This condition exposed employees to slips and falls from elevation.
