29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Suite 3142; The Administration Cubicle adjacent to the Deputy office had a relocatable power tap plugged into a pig tail extension cord, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (b) Suite 3142; The OPPD Office Director work space had a relocatable power tap plugged into a pig tail extension cord, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (c) Suite 3142; The Environmental Specialist workspace had a white relocatable power tap plugged into an unmounted cubicle power strip, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (d) Suite 3142; The Grant Processor/ Temporary Office had a white relocatable power tap plugged into a white extension cord, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords. This exposes employees to possible electrical fire hazards. (e) Suite 3290; Break Area/ Copy Room; A Simply Perfect microwave, Black and Decker coffee pot, and Sanyo refrigerator were plugged into relocatable power taps, violating the UL listing and labeling. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances. This exposes employees to possible electrical fire hazards.
