29 CFR 1910.23(a)(5):     Every pit and floor opening was not adequately guarded by standard railing or any other effected means:    a. At the south end of the mill, by the head saw, two by planks were laid over an opening to the chain sweep; the opening was not properly maintained to prevent an approximate 5 ft. fall such as by a secured flooring or railing or some other method.  An employee periodically walked out over the planks in order to maintain the equipment.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
