29 CFR 1904.4(a):     The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.     Located at 92 Newbury Street, Danvers, MA:    On or about February 6, 2013, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2013.    1. On or about January 17, 2013, an employee operating an electric powered pallet jack stepped off of while the powered pallet jack was still in motion and pinned left ankle between a storage rack upright and the powered pallet jack and as of February 6, 2013 had not returned back to work.    HOME DEPOT USA, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313794570, CITATION NUMBER 1, ITEM 1, ISSUED ON FEBRUARY 18, 2010, AND WAS AFFIRMED AS A FINAL ORDER DATE OF FEBRUARY 28, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 39 LONG POND ROAD, PLYMOUTH, MA 02360.
