29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: Terminal- On January 18, 2013, two empty circuit breaker slots were not covered by blankoff plates to shield live parts within the warehouse main panel box.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. Terminal Main Entrance, On January 18, 2013 the entrance railing lacked an intermediate rail on both sides of the platform.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. Terminal, On January 18, 2013, access and working space in front of the warehouse main electrical panel box was blocked by a large steel drum.
