Unless necessary for a use permitted in paragraph (g)(1)(i) of this section, flexible cords and cables shall not be used where run through doorways, windows, or similar openings, except as permitted in paragraph (a)(2)(ii)(1) of this section.    a. In apartment 223 where employees ran extension cords to power battery chargers, an extension cord was ran through a sliding glass door, thereby exposing employees to a contact hazard.     b. In apartment 223 where employees ran extension cords to power battery chargers, an extension cord was ran through a window, thereby exposing employees to a contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    a. In apartment 226 where employees ran an extension cord to power battery chargers, an extension cord plugs strain relief was pulled away from the cord exposing interior wires, thereby exposing employees to a contact hazard.

29 CFR 1926.501(b)(1):     "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a. On the 4th floor where employees were throwing trash over a balcony, employees did not have any fall protection, thereby being exposed to a fall hazard.
