29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      a) Warehouse: Employees that received forklift certification from their previous employers were not evaluated to determine their competency before operating the  Komatsu FG15S model FG15ST-2 serial 182758A forklift, on or about 1/25/13.

29 CFR 1910.305 (g)(1)(iv)(C)     Flexible cords and or cables were run through doorways, windows or similar openings:    a) Warehouse near scale: An extension cord, used to charge the engine in a 22 foot long box truck, was run to the outside under the emergency exit door, on or about 1/25/13.     b) Warehouse near center receiving door: An extension cord, used to charge the engine in a work van, was run to the outside under an overhead door, on or about 1/25/13.
