29 CFR 1910.304(g)(5):     The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:      On or about January 23, 2013, and at times prior thereto, in the receiving areas an extension cord with missing ground pin was used to provide power to other equipment at the store.  Employees were exposed to electrical hazards.

29 CFR 1910.305(a)(2)(x):     Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.        On or about January 23, 2013, and or at times prior thereto, in the receiving areas an electrical extension cord was observed being used under a closed steel overhead door.  This could damage the cord and expose employees to electrical hazards.

29 CFR 1910.305(b)(2)(i):     Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:      On or about January 23, 2013, and at times prior thereto, in the assembly areas an electrical light fixture did not have cover approved for the purpose intended to prevent accidental contact with live electrical parts exposing employees to an electrical hazard.
