29 CFR 1904.29(b)(6):     The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent, for a work-related contaminated needlestick injury:         a.     Establishment - A work-related needlestick injury which occurred on 11/5/12, was not entered on the OSHA 300 Form or equivalent as a privacy concern case.  The injured employee's name had been entered on the log.
